To request medical records from Coventry Emergency Squad, Inc. for yourself, a family member, or a client, kindly send an email to: ems@villageofnewberlinny.gov requesting a NYS HIPAA form, and include your mailing address. One will be mailed to you via USPS. Please fill out the form and return it to:
